Pelviscopic Treatment of Ovarian Tumor

Abstract
|
|
|
152 patients were sent to our department with the treatment of ovarian tumor, In our experience of mor than 500 cases were pelviscopic surgery especially pelvisopic treatment of benign ovarian cysts is a safe and effective technique. The accuracy
of
diagnosis of ovarian malignancy was preoperative expert ultrsonography, measurement tumor antigen(eg, Ca 125) and expert pelviscopic examination of the ovarian tumor. We were treated by pelviscopic operations ①cell washing,②careful inspection
of
pelvic upper abdomen, ③biopsy suspicious area, ④immediate frozen section, ⑤immediate staging laparotomy if cancer found. Use of the Nd: YAG LASER for pelviscopic surgery is a new procedure incorperating the Nd: YAG LASER dispersion effect 2~10
mm to
coagulate.
The surgeon fatigue has been reduced by the use of high resolution video cameras and monitors rather than the direct view through the endoscopy. We have continued to accumulated data on management of ovarian tumor ①puncture and fenestration(14),
②cystectomy and cyst preparation(66), ③oophorectomy(16), ④wedge resection and ovarian biopsy(28), ⑤salping-oophorectomy(28). 149 patients had successful pelviscopic removal of their adnexal mass. All of the mass were benign including,
①mucinous
cystqadenoma(17),②serous cystadenoma(31), ③serous cystadenofibroma(1), ④parovarian, ⑤functional ovarian cyst(40), ⑥cystic ter atoma(39), ⑦borderline tumor(1), ⑧endometroid adenocarcinoma(1), ⑨mucinous cyst-adenocarcinoma(1). There
patients
required laparotomy two for suspected carcinoma on frozen section. The procedure of removing a cystic mass through a colpotomy incision associated with puncture was used to avoid spillage of ovarian tumor. The purpose of this report is to
describe
our
technique and results of ovarian tumors with emphasis on the importnace of proper training, skill, and instrumentation.
